Location: Indianapolis, Indiana

Contract Length: 13 weeks
Job Description
- Provide physical therapy services in a home health setting with home visits and periodic office meetings.
- Average caseload: 5-6 patients per day, with 5-10 patients assigned daily equating to roughly 6-8 points per day.
- Types of cases: Start of Care and Follow Ups.
- Schedule: Monday to Friday, 8 AM to 5 PM.
- Weekend rotation: Approximately once per month.
- On-call requirement: Typically one week on-call every 4th week.
- Territory: Must be willing to travel up to 75 miles from the home clinic, averaging 200-250 miles per week.
- EMR system used: HCHB-Pointcare.
- Local candidates accepted at the same rate.
Required Qualifications
- Minimum 1 full year of recent Adult Home Health physical therapy experience (first-timers not accepted).
- Current Physical Therapy license valid in Indiana (license must be in hand; pending licenses accepted only if they can be obtained within a few days).
- CPR and BLS certification required (AHA, National CPR Foundation, or ARC accepted).
- Reliable transportation and current driver's license required.
- Agency-required liability insurance required.
Additional Requirements
- Must be willing to comply with background checks, drug screening, and health documentation including:
- Physical exam within 12 months of start.
- Negative TB test or chest X-ray within 12 months.
- Immunizations or titers for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella.
- COVID-19 vaccination or declination.
- 10-panel drug screen.
- Completion of facility-specific compliance training and policy acknowledgements prior to start.
- Must complete various state-specific background and fingerprinting requirements as applicable.
- Two professional references required.
- Verification of work history for at least the last 7 years or two most recent employers.
