Background Checks
- Must be completed within 45 days of start and updated annually
- Includes 7-year SSN trace with complete address history, alias verification, national and state sex offender checks, criminal searches, abuse registry, OFAC, FACIS III, federal criminal extended search, and international/statewide searches as applicable
- Alias Name Attestation required if applicable
Health & Immunization
- Physical exam within 12 months, signed by MD, DO, NP, APRN, or PA (Chiropractor not accepted)
- Negative TB test (QGold or Skin Test) within 1 year; chest x-ray if prior positive
- Required immunizations or titers:
- MMR (Measles, Mumps, Rubella) - 2 vaccines or positive titer
- Hepatitis B series or titer or declination
- Varicella - 2 vaccines or positive titer
- Flu vaccine required for 2025-26 season (no declinations accepted; exemptions only for medical/religious reasons with documentation)
- Drug testing within 45 days of start, including a comprehensive panel (amphetamines, barbiturates, benzodiazepines, cocaine, fentanyl, marijuana, opiates, etc.) with MRO signature
Competency Exams & Testing
- Medication Test/Pharmacology Competency Exam required
- Color Blind Testing within 12 months (Ishihara or online test)
- Competency Exam required
- Dysrhythmia/EKG Competency Exam required for all RNs (not applicable here but noted)
- Core Competency Exams (Parts I, II, III) with minimum 80% pass rate, updated annually
Additional Facility-Specific Requirements
- Photo for badge (passport style, solid background, no filters or facial piercings)
- Onboarding attestation and review of onboarding documents
