Compliance & Documentation Requirements

Background Checks

• Must be completed within 45 days of start and updated annually
• Includes 7-year SSN trace with complete address history, alias verification, national and state sex offender checks, criminal searches, abuse registry, OFAC, FACIS III, federal criminal extended search, and international/statewide searches as applicable
• Alias Name Attestation required if applicable

Health & Immunization

• Physical exam within 12 months, signed by MD, DO, NP, APRN, or PA (Chiropractor not accepted)
• Negative TB test (QGold or Skin Test) within 1 year; chest x-ray if prior positive
• Required immunizations or titers:

  • MMR (Measles, Mumps, Rubella) - 2 vaccines or positive titer
  • Hepatitis B series or titer or declination
  • Varicella - 2 vaccines or positive titer
  • Flu vaccine required for 2025-26 season (no declinations accepted; exemptions only for medical/religious reasons with documentation)
• Drug testing within 45 days of start, including a comprehensive panel (amphetamines, barbiturates, benzodiazepines, cocaine, fentanyl, marijuana, opiates, etc.) with MRO signature

Competency Exams & Testing

• Medication Test/Pharmacology Competency Exam required
• Color Blind Testing within 12 months (Ishihara or online test)
• Competency Exam required
• Dysrhythmia/EKG Competency Exam required for all RNs (not applicable here but noted)
• Core Competency Exams (Parts I, II, III) with minimum 80% pass rate, updated annually

Additional Facility-Specific Requirements
