Job Description

• Role: Case Manager RN
• Patient Ratio: 1:25-30 (Adults & Geriatrics)
• Schedule: 5 days per week, 8-hour shifts (8:30 AM - 5:00 PM, Monday-Friday) plus one full weekend every 4 weeks
• No call or holiday shifts required
• Responsibilities include initial patient assessments and follow-up visits at least every third day

Experience & Licensure Requirements

• Minimum 2 years recent acute care hospital Case Management experience
• Discharge Planning experience required
• Level 1 Trauma experience preferred
• Must have Epic experience
• Active Florida or Compact nursing license required
• Degree must be from an accredited school or approved by the Florida Board of Nursing
• Experience must be consecutive within the past 24 months in a U.S. hospital

Required Skills & Competencies

• Proficiency in Microsoft Office Suite, Cerner, CarePort, AllScripts, and Powerchart
• Ability to complete assessments and patient follow-ups as per facility standards

Prior Travel Experience

Certifications Required

• ACLS (Advanced Cardiac Life Support) – AHA or ARC only
• BLS (Basic Life Support) – AHA or ARC only
• PALS (Pediatric Advanced Life Support) – Required for certain units; check unit specifics
• NRP (Neonatal Resuscitation Procedure) – Required for specific units (PICU, NICU, L&D, Mother-Baby, Postpartum)
• Electronic Fetal Monitoring (EFM) – Required for Labor and Delivery and Antepartum specialties (AWHONN certification only)
• NIH Stroke Certification – Required for all RNs
• Medication Test/Pharmacology Competency Exam – Required for all nursing professions except Case Managers
• Annual Skills Checklist and other competency exams as applicable

Background & Compliance Requirements

• Background check within 6 months of start, including all locations lived/worked/schooled in past 7 years
• Fingerprinting: Florida AHCA Level II fingerprinting required prior to start, with results dated within 30 days of start
• Education verification from accredited institutions required; candidates from fraudulent nursing schools will be canceled
• Government-issued photo ID required during hours of service
• I9 and E-Verify documentation required
• Proof of name change affidavits reviewed on a case-by-case basis

Health & Immunization Requirements

• Physical exam within 6 months of start, updated annually
• Negative 10-panel drug screen within 12 months of start (instant tests not accepted)
• TB testing: QuantiFERON lab test within 6 months or chest X-ray if prior positive
• Annual TB questionnaire
• Immunizations:

  • 2 MMR vaccines or positive titer (boosters required if low/equivocal titer)
  • Hepatitis B series, titer, or signed declination
  • 2 Varicella vaccines or positive titer (boosters required if low/equivocal titer)
• Annual flu vaccination or declination required during flu season (Oct 1 - Mar 31)
• COVID-19 vaccination preferred (for tracking purposes)
• Annual fit testing for approved masks (3M 1860 Regular or Small) required; no onsite fit testing
