Arrange non-medical support services (e.g., housing, transportation) to ensure treatment plan compliance.
Coordinate care transitions with physicians, social workers, discharge planners, and claims professionals.
Engage specialty resources as needed to optimize patient outcomes.
Maintain detailed records of clinical, functional, and fiscal outcomes throughout case management.
Identify opportunities for health promotion and illness prevention.
Prevent adverse patient events when possible and intervene promptly if prevention is not feasible.
