RN: Case Manager - Contract Position
Location:
Kentucky, Lexington (Onsite)
Employment Type:
Temporary Contract
Contract Length:
13 weeks
Work Schedule:
- Monday through Friday, 8:00 AM to 5:00 PM
- On-call required: 1 weekend per month and 1 additional day each week
- 40 hours per week (not guaranteed)
Job Responsibilities
- Perform comprehensive assessments of high-risk members.
- Collaborate with primary care providers to develop and implement individualized, comprehensive care plans with targeted interventions.
- Continuously reassess and address cost-efficient, appropriate levels of care.
- Arrange non-medical support services (e.g., housing, transportation) to ensure treatment plan compliance.
- Coordinate care transitions with physicians, social workers, discharge planners, and claims professionals.
- Engage specialty resources as needed to optimize patient outcomes.
- Maintain detailed records of clinical, functional, and fiscal outcomes throughout case management.
- Identify opportunities for health promotion and illness prevention.
- Prevent adverse patient events when possible and intervene promptly if prevention is not feasible.
- Advocate for patients by protecting privacy and confidentiality.
Required Qualifications
- Valid Registered Nurse (RN) license in Kentucky.
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
- Minimum of 1 year of recent Case Management experience.
- Experience in a Home Health setting.
- Knowledge and experience with OASIS documentation system.
