Location: Rancho Cordova, California
Contract Length: 13 weeks
Shift: Days
Hours: 8 hours/day, 40 hours/week

Provide occupational therapy services in a home health setting.
Average of four visits per day, including evaluations and revisits.
Coverage area includes northeastern Sacramento, Folsom, and Woodland (Yolo County).
Schedule is Monday through Friday, 8:30 AM to 5:00 PM, with first-to-float assignment.

Minimum 2 years of occupational therapy experience in a healthcare setting.
Active California OT license.
Current Basic Life Support (BLS) certification.
National Provider Identifier (NPI) number.
Valid driver’s license and proof of automobile insurance.
Must provide two supervisory references at time of application (one within the last 12 months, one within the last 3 years).
Legal name on application must match government-issued ID (no middle initials or middle names).

Candidates with Certified Occupational Therapy Assistant (COTA) credentials and home health experience may be considered if no OT applicants are available.
