Travel RN - Care Coordination (Case Management)

Job Description

Seeking an experienced Core Coordinator RN (case manager) for a travel contract in an acute care hospital setting.

Requirements:

• Minimum 3 years hospital experience, including acute level care coordination
• Active California RN license and BLS certification
• Proficient with Midas, InterCall, Cerner, Curaspan, and Microsoft Office programs
• Experience with discharge planning and utilization review
• Prior acute care experience within CA Dignity/CommonSpirit facilities is mandatory
• Must provide 2 supervisory references (1 within past 12 months, 1 within past 3 years)
• Candidates cannot have been employed (full-time, part-time, or PRN) by any CommonSpirit-associated facility within the past year (PRN through outside firms is acceptable)

Work Details

• Patient assignment ratio: 1:20
• Shift length: 8 hours (option for 12-hour night shifts if candidate has ED background and is willing to work nights)
• No overtime available
• Weekend requirement: Every 3rd weekend
• Floating: Must cover all adult hospital areas except women's health (unless experienced)
• Orientation: 4 days webinar on charting + 2 days preceptor
• Dress code: Business casual conservative, no open-toed shoes, lab coats required, minimal to no perfume, clean professional appearance
• Must work 2 of the 3 major holidays (Thanksgiving, Christmas, New Year's); holiday time off considered but not guaranteed
• Excellent attendance, adaptability, and ability to quickly learn and perform on the floor expected
• Must take lunches and breaks

Additional Information

• 24/7 Respiratory Therapy and Pharmacy support available
• Manager is the charge nurse
• Profiles go directly to the unit for review
