Job Opportunity: Home Health Physical Therapist - Michigan

Job Summary

The Physical Therapist provides medically prescribed, goal-oriented therapy in accordance with the Physician Plan of Care, company policies, and professional Standards of Practice. This role involves patient assessments, development and implementation of physical therapy plans of care, documentation, patient and family education, and participation in interdisciplinary team meetings.

Essential Functions

  • Develop patient physical therapy plans of care.
  • Conduct patient visits for assessments and evaluations; communicate with physicians as needed.
  • Implement and document the plan of care accurately and timely.
  • Educate and instruct patients, families, and team members.
  • Participate in quality assessment and performance improvement programs.
  • Provide skilled restorative treatments including:
    • Pain reduction techniques
    • Use of moist heat whirlpool (portable), paraffin baths
    • Gait training, ultrasound, electrical muscle stimulation
    • Range of motion testing (goniometer)
    • Exercise program supervision
    • Prosthetic training, balance and coordination training
    • Cardiovascular rehabilitation
    • Home safety instruction
  • Perform initial and ongoing comprehensive patient assessments including OASIS.
  • Evaluate outcomes of care.
  • Supervise Physical Therapist Assistants, Certified Nursing Aides, and Home Health Aides; provide supervisory visits as necessary.
  • Follow agency policies and federal/state regulations.

Contract Length

  • Approximately 13 weeks (89 days rounded to nearest week).

Required Qualifications

  • Valid Physical Therapist license in Michigan.
  • Physical Therapist with at least 1 year of experience.
  • Physical Therapist experience within the last year.
