Required Qualifications

• Must Have:

  • 2 years of Acute Case Management experience for Case Managers
  • 1-2 years of Acute Social Worker experience (licensed in Texas) for Social Workers

Preferred Qualifications

• Master of Social Work (MSW)

Responsibilities

• Case Manager:

  • Provides case management services for inpatient and observation patients.
  • Conducts comprehensive assessments of psychosocial, medical, and discharge needs.
  • Coordinates the plan of care and collaborates with the multidisciplinary team.
  • Evaluates progression of care using InterQual.
• Social Worker:

  • Collaborates with RN case managers and the multidisciplinary team.
  • Performs assessments and develops discharge plans for high-risk patients.
  • Engages in consultation, education, counseling, advocacy, and referrals.
