RN - Case Manager Travel Position

Location: Wichita, Kansas
Employment Type: Travel
Contract Length: 13 weeks
Hours per Week: 36 hours

Unit Information

Unit: Case Management
Floor: CPCU (3rd floor) & PCU (4th floor) - Step Down Units
Beds: CPCU - 14 beds (cardiac step-down, CABG and TAVR patients), PCU - 14 beds (medical step-down with varied patients)
Patient Ratio: 1:14
Shift: Day shift, Monday to Friday, 8:00 AM to 4:30 PM (No weekends required)
EMR Systems: Meditech and Midas
Team: Collaborative and supportive case management team with consistent physicians and mid-level providers

Job Description

Perform case management assessments
Discharge planning
Work in a fast-paced, constantly changing environment
Engage with post-acute placements and cardiac rehab coordination

Required Qualifications

Valid RN license in Kansas
Acute Care Case Management experience within the last year (minimum 1 year)
BLS certification
De-escalation skills in a healthcare setting
Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
Experience with MEDITECH EMR system
