Minimum Requirements
<ul><li>Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.</li><li>Current Physical Therapy license in Washington state or a valid compact license granting privilege to practice in Washington.</li><li>Current BLS/CPR certification from AHA or ARC.</li><li>Valid driver’s license, current vehicle insurance, and access to a dependable vehicle.</li><li>Home health experience preferred.</li><li>Experience with HomeCare HomeBase preferred.</li><li>Ability to complete patient evaluations and develop plan of care within state-specific guidelines.</li><li>Ability to travel and visit patients across South King County service area. May be required to visit other nearby agency service areas as needed.</li><li>Must be able to participate in occasional weekend rotation and potential availability on observed holidays as needed. <ul><li>Facility type: Home Health.</li><li>Shift: Days.</li><li>Shift hours: 08:00–16:00 (M–F; client also notes M–F 8:00–4:30 schedule and a 36-hour weekly guarantee).</li><li>Coverage area: South King County, WA.</li><li>Weekend/on-call: No routine on-call requirement; schedule flexibility and participation in a weekend rotation may be required per client guidance.</li><li>Mileage reimbursed between visits from first patient to last patient.</li><li>May supervise PTAs and home health aides per agency policy and state regulation.</li></ul>

Job Responsibilities
<ul><li>Evaluate patients’ functional status and physical therapy needs and consult with physicians to develop plans of care in accordance with CMS documentation requirements.</li><li>Provide physical therapy services within state scope of practice, following the plan of care and coordinating with the interdisciplinary team.</li><li>Document progress notes per agency policy and complete patient evaluations and plan of care within state timelines. Report evaluation outcomes to nurse supervisor within 24 hours.</li><li>Treat patients to relieve pain, restore or maintain function using evidence-based techniques and acceptable standards of practice.</li><li>Educate patients, families, and caregivers regarding the plan of care, functional limitations, and progress toward goals.</li><li>Participate in staff meetings, committees, and provide in-service education as requested by agency leadership.</li><li>Prepare written discharge summaries and progress reports at least every 60 days, and perform other duties as assigned.</li></ul>

<ul><li>Adhere to agency Therapy Standards of Practice and maintain state licensure and required certifications through continuing education.</li><li>Follow agency policies for supervision of PTAs and home health aides in accordance with state regulations.</li></ul>
