Job Summary

• Provide direct patient care to hospice patients and supportive care to their families.
• Manage an interdisciplinary team including Licensed Practical/Vocational Nurses, Certified Nursing Assistants, and support staff.
• Case manage all assigned patient clinical records.
• Perform initial and ongoing patient assessments.
• Develop and implement Hospice Plan of Care in collaboration with physicians and interdisciplinary team members.
• Deliver comprehensive physical, psychosocial, educational, emotional, and spiritual services, including bereavement support.
• Report changes in patient condition to attending and hospice physicians.
• Maintain accurate and updated medical records, including assessments, plans of care, and interdisciplinary notes.
• Practice infection control and standard precautions.
• Monitor staff scheduling and participate in quality assessment and performance improvement programs.
• May participate in on-call schedule.
• Maintain knowledge of pain and symptom management in end-of-life care.
• Conduct routine supervisory visits with team members at least monthly.
• Ensure compliance with policies, procedures, and regulatory requirements.
• Maintain close contact with patients’ families to provide information, support, and continuity of care.
• Act as primary liaison between physician, patient/family, and hospice team.
• Educate patients, families, and caregivers on care needs including medication administration, dressing changes, oxygen use, and hospice philosophy.

Schedule

• Monday to Friday, 8:00 AM to 5:00 PM
• On-call approximately 1 night per week
• Estimated caseload: 5-6 patients per day
• Travel radius: up to 60 miles

Contract Length

• 13 weeks

Required Qualifications

• Valid RN license in Michigan
• Hospice experience of at least 1 year, including experience within the last year
• BLS certification issued by the American Heart Association
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• Experience in Acute Care setting
• Experience in Long Term Care setting
• Experience in Home Health setting
• Oncology experience
