Position Details

• Specialty: Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)
• Location: Jackson, Ohio
• Employment Type: Travel/Contract
• Contract Length: 13 weeks
• Shift: Nights, 3 shifts of 12 hours each (3x12s)
• Patient Ratio: 42:1
• Schedule Requirements: Every other weekend
• Floating: Not required
• First Time Traveler: Yes, acceptable
• Scrub Color: Any solid color scrubs
• Experience: Long Term Care experience preferred
• Licensure: Ohio LPN or compact LPN license required
• Certifications: Basic Life Support (BLS) required
• Charting System: Point Click Care (experience not required)

Compliance and Documentation Requirements

Licensure & Background Checks

• Active Ohio LPN or compact LPN license with no board actions
• 7-year criminal background check including:

  • SSN Trace
  • Counties lived/worked
  • Statewide, National Criminal, and National Sex Offender checks
  • Facility Level 3 and OFAC checks
• EPLS/GSA/SAM and HHS/OIG checks, updated monthly
• BCI and FBI fingerprinting within 12 months of start, updated every 5 years
• Education verification required

Certifications

• BLS (Basic Life Support) - AHA only, required for all candidates
• ACLS, PALS, NRP, CCRN, TNCC certifications are not required for this position but may be relevant for other specialties
• Crisis Prevention Institute (CPI) certification required only for Behavioral Health/Psych roles

Health Requirements

• Physical exam confirming clearance to work without restrictions, completed prior to start
• Fit mask test within 6 months of start (3M 1860 or 3M VFlex masks)
• Immunizations and titers:

  • Tetanus/Diphtheria/Tdap within 10 years
  • Negative TB test (Quantiferon Gold within 30 days preferred; Tspot not accepted)
  • Measles (Rubeola), Mumps, Rubella: Positive titer or 2 vaccines; booster or declination if negative/equivocal
  • Hepatitis B: Positive titer required or signed declination
  • Varicella: Positive titer or 2 vaccines; booster or declination if negative/equivocal
• Negative 12-panel drug screen within 30 days of start
• Seasonal flu vaccination or signed declination required for flu season (9/1/2025 – 3/30/2026)
• COVID-19 vaccination or signed declination required

Facility-Specific Documents & Training

• Recent passport-style photo for badge
• Completion of confidentiality, authorization to release information, contract employee information forms
• Review and acknowledgment of non-employee orientation packet and post-tests (including age-specific and cultural competency)
• Dress and appearance standards acknowledgment
• Commitments to care and code of conduct acknowledgments
• Flu and COVID-19 policies acknowledgment
• N95 beard waiver if applicable (only if notified by facility)
• Signed job description

History & Experience Verification

• Employment application including 3 years prior work history with at least 2 years clinical practice in acute care or similar area
• Two positive professional references related to similar experience (most recent employer preferred)
• Unit-specific skills checklist, updated annually
