Job Description
- Position: LPN Hospital Float
- Schedule: Every other weekend
- Float assignments include Burks, halls (subacute rehab), and Households units.
- Minimum 1 year of LVN/LPN experience required.
- Certifications required: ACLS & BLS.
Unit Descriptions
Household Units (HH) – Long-Term Care (LTC):
- Six units with 14 private bedrooms each.
- Central kitchen, den with books and TV, washer/dryer for resident clothing.
- HH 5-6 specialize in dementia or cognitive impairment care; staff must badge in/out to prevent patient exit.
- Staff must recognize individual patient preferences and routines to deliver personalized care.
Hall 1 – Subacute Care:
- 42-bed LTC unit with high complexity needs.
- Fast-paced environment with patients requiring post-op, post-stroke, post-cardiac care, ambulatory rehab, and wound care.
Hall 2 – LTC/Memory Care:
- 50-bed LTC unit with varying acuity levels from high functioning to bedridden.
- Common diagnoses: diabetes, cardiac issues, dementia, behavioral conditions.
Burk 1 – Pulmonary Care:
- 17-bed LTC unit for chronic and acute illness care.
- Includes chronic trach patients.
- Variety of bed types to meet physical needs.
Burk 2 – Ventilator Unit:
- 18-23 beds for patients with chronic and complex respiratory problems.
- Many residents require ventilator support.
- Requires strong organizational and leadership skills to assess patient status, implement nursing interventions, and develop care plans.
- Special training on ventilator care and alarm response is required.
Experience Requirements
- Minimum 1 year of LVN/LPN experience.
- Prior travel experience is not explicitly required.
Certifications & Licenses Required
- ACLS (Advanced Cardiac Life Support) – AHA Provider Certification or equivalent.
- BLS (Basic Life Support) – AHA Provider Certification or equivalent.
- Valid state LVN/LPN license with primary source verification.
- NURSYS verification as applicable.
- Additional certifications may be required based on unit assignment (e.g., Dysrhythmia/EKG competency, Medication Test/Pharmacology Competency Exam).
Compliance & Documentation Requirements
- Background check within 6 months covering all locations lived/worked in the past 7 years.
- Education verification and transcript/diploma preferred.
- Physical exam within 1 year confirming ability to meet essential job functions.
- Negative TB test or chest X-ray within 1 year.
- Drug screening (9 panel) within 30 days of start.
- Immunizations/vaccinations including MMR (Measles, Mumps, Rubella), Hepatitis B, Varicella, and seasonal flu vaccination or approved exemption.
- Respirator fit test (for clinical staff) completed prior to start.
- Completion of facility-specific policies and acknowledgments (e.g., Floating Policy, Time and Attendance Policy).
- Unit-specific competency exams and skills checklists as applicable.
- EKG/Dysrhythmia competency exam with limited attempts and remediation process if failed.
- Stroke and Trauma CEUs may be required depending on unit and length of assignment.
Additional Notes
- Candidate must be able to badge in/out on dementia units.
- Must be able to provide personalized care based on patient preferences and routines.
- Training on ventilator care and alarm response is mandatory for ventilator unit assignments.
- Physical and mental ability to work in fast-paced and complex care environments is essential.
