Job Opportunity: Pharmacist Outpatient (Travel/Contract)

Required Experience & Qualifications

• Minimum 2 years of experience in pharmacy
• Required specialty experience: Sterile Compounding IV room experience with hazardous and non-hazardous drugs
• PharmD degree required
• California Pharmacist license required
• First-time travelers are eligible if experience requirements are met
• Prior travel experience: Not explicitly required

Required Certifications & Licenses

• Valid California Pharmacist license
• Current and valid Driver’s License or government-issued photo ID
• Background checks including Social Security Trace, National Criminal History, Sex Offender Search, FACIS level 3, EPLS/GSA/SAM, HHS/OIG (all within 30 days of start)
• Education verification (Diploma or official transcripts)
• Other certifications typically required for pharmacists (verify with recruiter if applicable)

Health & Compliance Requirements

• Physical exam within 1 year of start, updated annually
• Fit mask testing (specific N95 masks required, annual expiration for Aya candidates)
• Immunizations or proof of immunity for:

  • Tetanus/Diphtheria/Tdap (declination accepted)
  • Negative TB test (QGold within 1 year)
  • Measles, Mumps, Rubella (positive titer or proof of vaccination)
  • Hepatitis B (titer, declination, or completed vaccine series)
  • Varicella (positive titer or proof of vaccination)
• Drug screening (9 panel within 12 months)
• Seasonal flu vaccination or signed declination (required during flu season)
• COVID-19 vaccine or declination (vaccine must be after 9/12/2023; monovalent Pfizer/Moderna or 2 doses Novavax accepted)

Facility-Specific Requirements

• Completion of UCLA-specific onboarding documents and training modules including:

  • HIPAA training
  • Annual OSHA and education modules
  • Confidentiality agreement
  • Facility-specific abuse reporting and safety modules
  • Photo for badge (high-quality, recent, professional)
• References: 2 required, supervisor level or above, one within last year
• Employment application covering last 7 years with explanations for gaps
• Work history verification for last 5 years

Additional Notes

• Candidate must be prepared to complete all required background checks, health screenings, and facility-specific onboarding prior to start
• Floating to sister facilities may be required based on client needs
• No weekend or on-call shifts required for this position
