Position Details

Experience & Certification Requirements

Compliance & Documentation Requirements
- Background Checks:
- Must be within 45 days of start, covering all locations lived/worked in past 7 years
- Includes SSN trace, Death Master Index, Nationwide and County checks, Sex Offender, FACIS Level 3, Education Verification, MVR (required for Home Health)
- Oregon Abuse/Neglect search required only for specific facilities (not applicable here)
- State-Specific Checks:
- Washington WATCH Search required for placements with WA licensure or compact license (tracking only for this Oregon placement)
- Health Documents:
- Physical exam within 1 year of start, signed by MD, NP, PA, or DO
- Fit mask test required for travel starts on or after 2/5/2024, updated annually
- Immunizations/vaccinations required: TDAP, MMR (Rubeola, Mumps, Rubella), Hepatitis B (or declination), Varicella, Flu seasonal vaccine (or declination), COVID vaccine or declination
- Negative TB test or chest X-ray within 12 months, updated annually
- 10-panel drug screen within 90 days of start (instant tests not accepted)
- References:
- Minimum 2 references covering 1 year of employment within last 3 years in comparable specialty/setting
- Employment Application:
- Must include at least last 5 years of employment with month/year format

Additional Notes
- Traveler must comply with Providence Health and Services policies including float policy acknowledgment (may be required to float to other Providence facilities within reasonable distance)
- Facility requires COVID vaccination primary series plus booster
- Traveler must have reliable transportation due to travel between patient homes
- Facility groups clients to minimize drive time
This position is ideal for LVNs/LPNs with at least 1 year of skilled home health experience who are comfortable with day shifts, have BCLS certification, and are ready to start ASAP in the Portland, OR area. Prior travel experience is not required, and first-time travelers are welcome.
