Job Details
- Contract Length: 13 weeks
- Employment Type: Travel/Contract
- Setting: Hospital, adult care management teams
- Schedule: No weekends or holidays required
- Role Overview:
- Backfill position for RN Case Manager
- Service-based case management following provider teams
- Case Management Assistants support CarePort referrals
- Patient ratio approximately 1:28
- Full-time floats cover vacations, call-offs, PTO, and scheduled days off
Experience Requirements
- Minimum 2 years of care management experience with discharge planning in a hospital setting
- Experience with CarePort (WellSky) and EPIC systems required
- First-time travelers accepted
- Prior travel experience is NOT required
Certifications Required
- BLS (Basic Life Support) – AHA only, no certificates of completion accepted
- ACLS (Advanced Cardiac Life Support) – AHA only, required as applicable
- PALS (Pediatric Advanced Life Support) – Required only for specific pediatric units (not required for this adult assignment)
- NRP (Neonatal Resuscitation Program) – Required as applicable
- Certifications must be current with no lapses during assignment
Compliance & Documentation Requirements
- Government-issued photo ID (Driver’s License/State ID)
- Background checks including:
- 7-year background check within 30 days of start, renewed annually
- EPLS/GSA/SAM and HHS/OIG checks, updated annually
- Employment application and 2 recent professional references (from managers/supervisors)
- Physical exam within 1 year of start, updated annually
- Fit mask test within 1 year (N95 or equivalent)
- Negative TB test or chest X-ray within 1 year; if past positive, must complete annual TB questionnaire
- Immunizations or titers for:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B (titer, series, or declination)
- Varicella
- Negative 10-panel drug screen within 30 days of start, updated annually
- Seasonal flu vaccination required year-round with strict deadlines; no exemptions allowed
- Facility-specific documents and attestations to be reviewed and signed prior to start (e.g., confidentiality, code of conduct, sexual harassment policy)
- Pass/fail competency testing (EKG/medication) required for certain units; candidates must prepare accordingly
Additional Notes
- Candidates must submit all required orientation paperwork within 3 weeks of start date to avoid removal from assignment
- Badge and pager return policies apply at assignment end to avoid fees
- This position is for adult case management teams; pediatric certifications only required if assigned to pediatric units
