OT Home Health - Travel/Contract Position
Location: San Ramon, California
Contract Length: 13 weeks
Job Details
- Specialty: Therapy/Rehabilitation (Occupational Therapy)
- Setting: Home Health
- Shift: Day shift, 8 hours per day (8:00 AM - 4:30 PM), 5 days a week
- Weekly Hours: 40 hours (Guaranteed 36 hours weekly, with expectation to see 5-6 patients per day)
- Patient Load: May visit up to 6 patients per day; standard productivity is 27 points
- Start Date: ASAP (Compliance process takes about 2 weeks)
- Weekend Work: Optional weekend patient visits to meet productivity goals
- On-Call: Not required
- Locals Accepted: Yes
- Transportation: Must have own transportation; occasional travel radius up to 50+ miles
- Documentation: Timely communication and documentation required; documentation tools provided and must be returned
- Attire: Any color scrubs
- Parking: Free parking available
- Mileage Reimbursement: IRS standard
Required Qualifications
- Valid Occupational Therapist license in California
- Minimum 1 year of Occupational Therapist experience within the last year
- Experience in a Home Health setting
- Current Basic Life Support (BLS) certification (AHA or ARC only)

- Government-issued photo ID (Driver’s License/State ID)
- Emergency Contact Form
- I9 Attestation
- Background Checks including 7-year work/living history, EPLS/GSA/SAM, and HHS/OIG verifications
- Physical Exam within 1 year, cleared to work without restrictions
- Immunizations and Titers:
- Tetanus/Diphtheria/Tdap (within 10 years or annual declination)
- Measles, Mumps, Rubella (2 vaccines or positive titers within required timeframes)
- Hepatitis B (titer, vaccine series, or declination)
- Varicella (2 vaccines or positive titer)
- Drug Screening: 10-panel drug screen within 1 year
- Flu Vaccine or Declination (required year-round)
- COVID-19 Vaccine documentation or declination form (religious/medical exemptions accepted)
- Auto Insurance and Auto Registration (must be current throughout assignment)
- References:
- 2 professional references for travel/allied/per diem acute care
- 1 professional reference for per diem non-acute care
- Work History Verification: 1 year employment verification related to profession
- Annual Skills Checklist and Competency Exams as applicable
Additional Notes
- The system groups clients to minimize drive and windshield time
- Training provided for documentation systems (MediScrib & OASIS)
- Guaranteed hours will not apply during the first 2 weeks (ramp-up, holidays, time off)
