Setting and coverage
- Setting: Home Health
- Territory: South and Southwest Indianapolis and surrounding areas
- Travel: 100–200 miles per week
- Coverage radius: Must be willing to travel up to 75 miles from home clinic
- Patients/day: 5–6
- Productivity expectation: 30 points per week
- Cases: Start of Care and Follow Ups
Required qualifications
- Valid Physical Therapist license in Indiana
- Physical Therapist (1 year of experience)
- Physical Therapist experience within the last year
- One completed travel assignment
- BLS issued by American Heart Association or American Red Cross
- Experience working with Adult patients
- Experience in a Home Health setting
- Driver's License
- Experience with Homecare Homebase
Schedule details
- Weekend rotation: every 3 weeks, on call for ORTHO only
- On-call: every 3–4 weeks
