Overview

Travel/Contract assignment in Hazard, Kentucky
13-week contract
3 South Stepdown unit
Day shift: 7:00–19:30
36 hours per week
1:4/5 ratios
Every other weekend required
Float required within scope of practice
Blocked scheduling cannot be accommodated

Required Qualifications

Associate of Science in Nursing / Associate Degree in Nursing
Valid RN license in Kentucky
Stepdown (2 years of experience)
Stepdown experience within the last year
ACLS - Advanced Cardiac Life Support issued by American Heart Association
BLS issued by American Heart Association
NIHSS
Driver's License
Flu vaccine required; declinations not accepted
Must reside 75 miles from the facility
2 references within 1 year of start date, with at least one from a supervisor or higher
7-year work history verification
Medication Test/Pharmacology Competency Exam
Competency Exam within 1 year prior to start date, renewed annually
Dysrhythmia/EKG Competency Exam within 1 year of start, renewed annually
Core Part I
Core Part II
Core Part III
Orientation Safety Manual Acknowledgement
Physical exam or physician note/statement within 1 year prior to start, showing clear to work and free of restrictions
Fit mask completion onsite
Tetanus/Diphtheria/Tdap within 10 years
Negative TB or Chest X-Ray within 1 year of start, renewed annually
Rubeola, Mumps, Rubella, and Varicella immunity documentation
Hepatitis B documentation
Drug testing within 30 days of start
West Virginia APS/CPS search results
Kentucky Vulnerable Adult Maltreatment Registry verification within 30 days prior to start

Allied state license hard copy, if available

Notes

Medical and religious flu vaccine exemptions may be approved case by case
Assignment confirmation must be loaded within 24 hours of accepting the offer
Start date on the job is the start date needed
No timecard edits after the pay period closes
