Operating Room RN Travel Contract

Overview

Required Qualifications

• Valid RN license in Indiana
• Associate of Science in Nursing / Associate Degree in Nursing
• Operating Room experience within the last year
• 1 year of Operating Room experience
• Active state license for the role, with primary source verification
• BLS certification
• ACLS certification
• PALS certification
• NURSYS verification
• Background check, including:

  • 7-year background check
  • State Medicaid
  • SSN verification
  • Education verification
  • National Sex Offender Registry
• EPLS/GSA/SAM check
• HHS/OIG check
• Orientation safety manual acknowledgement
• Competency exam
• Medication test/pharmacology competency exam
• 2 references within 3 years of start

Health and Onboarding Requirements

• Physical exam within 30 days of start
• Negative TB test or chest X-ray within 1 year of start
• Drug screen within 30 days of start
• Measles, mumps, rubella, hepatitis B, and varicella immunity documentation or acceptable vaccination/booster evidence
• Seasonal flu vaccination or approved medical/religious exemption during flu season
• Respiratory safety clearance
• Passport-style photo for badge
• Parking permit application if parking onsite

Additional Notes

• N95 fit testing is completed during orientation
• Onboarding attestation must be completed within 48 hours of candidate signing
