Overview
- Travel/contract role in Portland, Oregon
- Home health setting
- Start: ASAP
- 13-week contract
- Day shift, 8 hours per day
- Facility does not accommodate 4x10 or 3x12 schedules
- First-time travelers accepted
- Local candidates accepted within a 50–75 mile radius
- No weekend requirement
- No on-call requirement
- No floating between like units
- Block scheduling not available
Required Qualifications
- Valid RN license in Oregon
- Associate of Science in Nursing / Associate Degree in Nursing
- 1 year of Home Health experience
- Home Health experience within the last year
- BLS issued by American Heart Association or Heart and Stroke Foundation
- Experience with OASIS
- Own transportation
- Primary series COVID vaccination plus one booster
Schedule and Work Expectations
- May visit up to 6 patients per day
- Standard productivity: 30 points
- Occasional daily travel radius of roughly 60 miles, with 60–90 minutes total drive time
Compliance and Pre-Start Requirements
- Background check
- Education verification
- Employment application covering at least the last 5 years
- Minimum of 2 references covering 1 year of employment total within the last 3 years in a comparable specialty/setting
- Physical exam within 1 year of start
- Drug screen within 12 months of start
- TB screening or chest X-ray
- Measles, mumps, rubella, hepatitis B, varicella, tetanus/diphtheria/Tdap, flu, and COVID documentation or declination as applicable
- Respirator clearance
- State-specific Washington border-facility search requirements apply

- Charting includes EPIC Remote Client for Home Health, Rover, and Oasis
- Parking is free
- Mileage reimbursement follows the IRS standard
