Valid government-issued ID
Background check: 7-year criminal history within 30 days of start
Education verification of highest completed degree (transcript acceptable if verification unavailable)
Statewide criminal background clearance (iCORI) within 30 days prior to start
Physical exam within 12 months of start, including physician statement of fitness without restrictions
Drug testing within 30 days of start
Immunizations or valid declinations required for:
- COVID-19 (most recent vaccine or declination)
- Flu seasonal vaccine or declination (required during flu season)
- Tetanus/Diphtheria/Tdap
- MMR (Measles, Mumps, Rubella)
- Varicella
- Hepatitis B
Tuberculosis screening: Two PPD tests or equivalent within required timeframes; chest x-ray if positive
References: Two within three years of start
Employment history: 7 years with explanations for gaps over 30 days
