Job Opportunity: Physical Therapist - Home Health (Travel/Contract)

Location: Houston, Texas
Contract Length: 7 weeks

Position Overview

• Specialty: Therapy/Rehabilitation (Physical Therapy)
• Setting: Home Health
• Start Date: ASAP
• Average Patients per Day: 4-7 (depending on Start of Care visits)
• Types of Cases: Start of Care, Follow Ups, All Geriatric
• Guaranteed Hours: 40 hours per week (provided no declined visits or call-offs)
• Weekend Requirement: None
• Territory Coverage: Approximately 75-mile radius from Huntsville to North Houston
• Mileage Reimbursement: Yes, at IRS standard rate
• Locals Accepted: Yes
• Pending License Accepted: Yes
• Documentation: Must be completed same day
• Charting Systems: HCHB and Oasis
• Dress Code: Scrubs or Business casual
• Traffic Note: Be comfortable with Houston traffic density

Required Qualifications

• Valid Physical Therapist license in Texas
• Minimum 2 years of Physical Therapist experience
• Physical Therapist experience within the last year
• Basic Life Support (BLS) certification
• Experience working with Geriatric patients

Required Documentation and Compliance

• Government-issued photo ID (Driver’s License/State ID)
• Background Check (7-year search including SS Trace, County, Nationwide, Sex Offender, Motor Vehicle; updated annually)
• EPLS/GSA/SAM and HHS/OIG checks (annual)
• Certifications:

  • BLS (Basic Life Support)
  • ACLS and PALS recommended but not explicitly required
• State License Verification (primary source online verification)
• Competency Exams as applicable (Medication Test/Pharmacology, general competency)
• Facility-specific training: HCHB training required if no experience within last 6 months (self-paced online course with assessment)
• Physical Exam within 1 year of start
• Negative TB test or Chest X-Ray within 1 year (with specific guidelines for past positives)
• Immunizations or proof of immunity:

  • Measles (Rubeola), Mumps, Rubella (MMR)
  • Hepatitis B (titer, declination, or vaccine series)
  • Varicella (titer, vaccines, or history of chickenpox)
• Drug Testing: 9-panel drug screen within 1 year of start
• Flu Vaccination or Declination during flu season (Oct 1 - Mar 31)
• COVID Vaccination or Declination required
• References:

  • 2 professional references for travel, allied, or per diem acute care
  • 1 professional reference for per diem non-acute care

Additional Notes

• Candidates should be comfortable with the travel territory and traffic conditions in Houston.
• Documentation and charting are expected to be completed the same day.
• Mileage reimbursement is provided for travel within the territory.
• Pending licenses are accepted, allowing for some flexibility in licensure status at start.
