Job Details

• Specialty: Therapy/Rehabilitation (Physical Therapist Assistant - PTA)
• Setting: Home Health
• Schedule:

  • 5 days per week, 8 hours per day (8:00 AM – 4:30 PM)
  • No weekends, no on-call
  • Office closed on major holidays
• Patient Load: Approximately 5 patients per day (30 points per week)
• Travel: 45-60 miles driving per day covering Newport News and Yorktown areas
• Start Date: ASAP

Required Qualifications

• Valid Physical Therapy Assistant license in Virginia
• Minimum 1 year of Physical Therapy Assistant experience within the last year
• Basic Life Support (BLS) certification required
• Experience in a Home Health setting

Compliance and Documentation Requirements

• Background check: 7-year criminal history check covering all locations lived/worked in past 7 years
• DMV driving record from state of license issuance required
• Virginia State Police Check required prior to start
• Physical exam within the last 12 months
• Negative TB test or chest X-ray per facility protocol
• Drug screen within the last 12 months
• Immunizations/vaccinations required or documented exemption for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
  • Seasonal Flu (required during flu season with deadlines for documentation/exemptions)
• COVID vaccine series preferred
• Professional references: 2 required
• Employment application/resume required
• Annual updates required for skills checklist and certain competencies

Orientation and Training

• Completion of facility-specific confidentiality, dress code, safety, drug-free, and hand hygiene policies required prior to start
• Facility-specific competency exams and medication/pharmacology tests as applicable
• Pass/fail computer-based learning (CBL) tests during orientation; multiple attempts allowed

Additional Notes

• No requirement to report to a home base before or after shifts
• Mileage reimbursement provided at IRS rate
• Photo for facility ID badge required (recent, professional, and meeting specific guidelines)
