Job Description

Seeking a Physical Therapist for an outpatient setting.

Required Qualifications

• License: Valid Physical Therapist license for Massachusetts (primary source verification required; conditional licenses not accepted)
• Certifications:

  • BLS (Basic Life Support) certification required prior to onboarding (AHA, Red Cross, CHSF, or Military accepted)
• Experience:

  • Minimum years of experience in specialty: Not explicitly stated; assume standard for outpatient physical therapy roles (confirm with recruiter if needed)

Health and Safety Requirements

• Documentation of immunizations or immunity for:

• Criminal background check within 45 days prior to start and updated if >6 month break
• Massachusetts CORI background check within 30 days prior to start and updated if >6 month break
• Social Security Number trace for past 7 years
• National Sex Offender Registry check within 30 days prior to start and updated annually
• Exclusion checks (OIG, GSA, MassHealth, NH Medicaid) within 30 days prior to start and annually
• Education verification of highest degree achieved
