Required Credentials & Certifications

• Licenses:

  • Active nursing license verified through primary source within 30 days of start
  • Nursys verification required (within 30 days of start)
• Certifications:

  • BLS (AHA or ARC only)
  • ACLS (AHA only)
  • PALS (AHA only)
• Professional Documentation:

  • Resume/Profile submission required
  • Criminal background check completed within 30 days prior to start, including SSN trace, county and national checks, education and employment verification (last 5 years), and driving record
  • HRIS form submission required
  • Badge photo (forward facing, well-lit, neutral background, JPEG)
  • Contract annual education packet completion
• Competency & Skills Testing:

  • Skills checklist specific to scope of practice, updated annually
  • Competency exam with passing score of 80% or higher within one year prior to start
  • Core competencies (HIPAA, Safety, Infection Control, etc.) with passing score of 80% or higher, updated annually

Health & Immunization Requirements

• Amsler Grid Vision Test
• COVID-19 vaccine or exemption accepted
• MMR: Proof of 2 immunizations, positive titer, or declination with signed statement
• Fit Test: N95 mask fit test within 1 year prior to start, updated annually
• Hepatitis B: Proof of 3 immunizations, positive titer, or declination with required approvals
• Influenza vaccine during flu season (10/1 - 4/30) or medical/religious exemption with proper documentation
• TB Test: Negative QuantiFERON blood test within 1 year or 2-step skin test; if positive, MD note or proof of treatment required
• Health statement signed within 1 year confirming ability to work without restrictions
• Varicella: Proof of 2 immunizations, positive titer, or declination with signed statement
• Tdap: Proof of vaccination within past 10 years or declination with required approvals
• Annual TB Questionnaire within 6 months of start, with follow-up documentation if positive
• Eye Exam & Color Vision: Ishihara Color Test and Snellen Vision test within 1 year prior to start
• Drug Screen: Negative 10-panel drug screen within 30 days prior to start
