Job Opportunity: Behavioral Health LVN / LPN

Job Details

• Shift: Night shift, 5 days per week, 8 hours per day (11:00 PM – 7:30 AM)
• Patient Population: Adult psychiatric patients with diagnoses including ADHD, Bipolar Disorder, Autism Spectrum Disorder, Eating Disorder, PTSD, Anxiety Disorder, Depression, Major Depressive Disorder, Psychotic Disorder, Oppositional Defiant Disorder
• Additional Patient Conditions: Substance abuse disorder, alcohol or opiate withdrawal (usually stable on admission), hypertension, diabetes, geriatric patients at high fall risk, patients in pain, occasional patients with wounds
• Unit Features:

  • Use of electronic patient rounding
  • Rover system for taking pictures, entering vital signs, and food intake
  • Special equipment: ObservSmart

Required Qualifications

• Valid LPN / LVN license in California
• Minimum of 1 year of Behavioral Health experience within the last year
• Basic Life Support (BLS) certification issued by American Heart Association or American Red Cross
• Certification in Management of Assaultive Behavior (MAB), CPI, MOAB, ProAct, PMAB, or Handle With Care
• Experience with Epic

Background and Compliance Requirements

• 7-year background check including SSN trace with all aliases, county searches, nationwide sex offender search, education verification, and FACIS Level 3 screening (initial and monthly updates)
• EPLS/GSA/SAM and HHS/OIG checks required and updated annually
• Education verification of highest level achieved (not required if highest education is high school)
• Sex offender search within 90 days of start
• Drug screening: Negative 9-panel drug screen within 6 months (excluding marijuana/cannabinoids due to California law)
• Employment application with full 7 years of work history, no gaps, and employment verification for up to 7 years
• Professional references: 2 for clinical/allied/per diem acute care candidates; 1 for per diem non-acute care

Health and Immunization Requirements

• Physical exam within 1 year of start (must indicate cleared for work)
• Fit mask test within 1 year, renewed annually (specific mask types required)
• TB screening: Negative QGold or TSPOT within 90 days or 2-step PPD testing with annual renewal
• Immunizations or titers required:

  • Tetanus/Diphtheria/Tdap (within 10 years)
  • Measles (Rubeola), Mumps, Rubella (MMR)
  • Hepatitis B series or signed declination
  • Varicella (chickenpox)
  • Seasonal flu vaccination during flu season (Nov 1 – Apr 30) or signed declination
• Color blindness exam (in-person, by licensed healthcare provider)
• Respiratory safety clearance prior to start, renewed annually

Competency and Testing Requirements

• Medication test/pharmacology competency exam as applicable
• Unit-specific skills checklist, renewed annually
• Dysrhythmia/EKG competency exam may be required for some telemetry units
• Pass/fail testing (e.g., EKG exam) may be required onsite after start for ICU and telemetry nurses (verify applicability)

Facility-Specific Documents and Training

• Review and sign facility-specific job description, standards of business conduct, code of conduct, vendor training, overpayment refund policy, and general regulatory orientation prior to start
• Complete conditional offer of employment attestation prior to start
• TB questionnaire signed by candidate and healthcare provider within 90 days, renewed annually
• Religious accommodation forms required if requesting exemptions for facial hair or certain vaccines

• No return to office (RTO) accepted for this position
• Prior travel experience is not explicitly required
