Physical Therapist - Home Health (Travel/Contract)
Location: San Jose, California
Contract Length: 16 weeks (approximately 4 months)
Job Overview
- Specialty: Therapy/Rehabilitation (Physical Therapist)
- Employment Type: Travel/Contract
- Shift: Day shift, Monday to Friday, 8:00 AM – 5:00 PM (5x8 hours)
- Hours: 40 hours per week guaranteed
- Patient Load: Minimum 23-24 visits per week required
- Patient Population: Home Health, Wound Care, Geriatric Home Health Patients
- Weekend Work: Not required
Required Qualifications
- Valid Physical Therapist license in California
- Minimum 1 year of Physical Therapist experience, including experience within the last year
- Experience in a Home Health setting
- Basic Life Support (BLS) certification
Job Expectations & Qualities Valued
- Patient care focused

- Self-starter with ability to complete charting efficiently
- Charting must be completed onsite using provided tablet (no remote/charting from home)
- May be required to travel to surrounding counties (details discussed during interview)
- Locals within a 50-mile radius are eligible
Compliance & Documentation Requirements
- Government-issued photo ID (Driver’s License or State ID)
- Background checks including SS Trace, County searches (last 7 years), Nationwide, Sex Offender Registry, and annual federal exclusions (EPLS/GSA/SAM, HHS/OIG)
- Physical exam within 6 months signed by MD/DO confirming freedom from active communicable/infectious diseases
- Immunizations or proof of immunity required for:
- COVID-19 (vaccine or approved medical/religious exemption)
- Influenza (vaccine or declination form during flu season)
- Tetanus/Diphtheria/Tdap (within 10 years or exemption)
- Measles, Mumps, Rubella (proof of immunity or vaccination per birth year guidelines)
- Hepatitis B (titer, vaccine series, or declination)
- Varicella (titer, vaccination, history of chickenpox, or declination)
- Negative TB test or chest X-ray within 1 year (with special rules for past positives)
- 9-panel drug screen within 1 year of start
- Auto insurance card required
- Emergency contact form required prior to start
- References: 2 professional references for travel/allied/per diem acute care roles (within 3 years)
Additional Information
- Mileage reimbursed at IRS standard rate for travel between patients (not to/from home)
- Free residential parking available
- Dress code: Any color scrubs and closed-toe shoes
- On-call is discretionary but not required
- All required compliance documents must be submitted before starting patient care; compliance processing may take 2-3 weeks
- Requests for time off (RTO) must be submitted with application; no additional RTO accepted after offer
