Job Overview
- Travel/contract physical therapy role in home health
- Start: ASAP
- Shift: Day, 5x8 hours
- Guaranteed hours: 36 per week
- Contract length: 13 weeks
- First-time travelers not accepted
- Locals accepted
- On-call not required
Location
- San Ramon, California
- Service area includes Santa Clara, Sunnyvale, Cupertino, Milpitas, Mt. View, Campbell, Los Altos, Saratoga, Los Gatos, and San Jose
Schedule and Caseload
- 8-hour day shifts
- May visit up to 6 patients per day
- Standard productivity: 27 points
- Weekend visits may be used to meet productivity
Required Qualifications
- Valid Physical Therapist license in California
- Physical Therapist (1 year of experience)
- Physical Therapist experience within the last year
- One completed travel assignment
- ACLS issued by American Heart Association or American Red Cross
- BLS issued by American Heart Association or American Red Cross
- Experience in a Home Health setting
Work Expectations
- Own transportation required
- Timely communication and documentation required
- Documentation tools are provided and must be returned
