Job Description

• Provide and coordinate home nursing care according to physician orders and agency policies.
• Directly supervise and evaluate Home Health Aides and other Licensed Practical/Vocational Nurses.
• Conduct home visits with an average patient load of 5-8 patients per day.
• Types of visits include Start of Care, Wound Care, Follow-up Visits, etc.
• Must have reliable transportation, a current driver’s license, and required liability insurance.
• Travel radius up to 75 miles from the home clinic; approximately 250-300 miles traveled weekly.
• Use of electronic medical records system HomeCare HomeBase (HCHB) required.
• Schedule is typically Monday through Friday, 8 AM to 5 PM; weekend and on-call requirements vary and should be discussed during the interview.
• Dress code: Scrubs.

Required Qualifications

• Minimum 1 full year of recent Adult Home Health or Hospice experience in the specialty.
• CPR and BLS certification required (AHA, National CPR Foundation, or ARC accepted).
• Must have an active and valid LPN/LVN license for the state of Maine (license in hand, no pending licenses accepted).
• Proficiency with HomeCare HomeBase (HCHB) within the last 6 months required.
• First-time travelers not accepted; recent specialty experience is mandatory.
• Must have reliable transportation and meet agency insurance requirements.

Additional Requirements

• Physical exam within 12 months prior to start, confirming good health and no communicable diseases.
• Negative TB test or chest X-ray within 12 months, updated annually.
• Proof of immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella as per standard healthcare requirements.
• Drug screening (10-panel) required.
• Seasonal flu vaccination or documented declination required.
• COVID-19 vaccination series or declination required.
• Background checks including 7-year history, sex offender search, and state-specific screenings as applicable.
• Education verification and work history verification (minimum 2 most recent employers or last 7 years).
• Various compliance documents and policy acknowledgments must be reviewed and signed prior to start.
• Personal or auto insurance policy required for home health placements, with candidate listed as insured or driver.

Travel Experience

• Prior travel experience not explicitly required, but must have recent specialty experience in home health or hospice.

Schedule & Workload

• Typical schedule: Monday to Friday, 8 AM to 5 PM.
• No guaranteed hours; patient census may fluctuate.
• Weekend and on-call requirements vary by location and should be clarified during interview.
• Patient visits and documentation are point-based, with an expectation of approximately 30 points per week.
