Shift: Days, Monday through Friday, 8:00 AM – 4:30 PM (5x8 schedule)
Hours: 40 hours per week

Job Description

• Inpatient Case Manager role focusing on medical home care coordination, placement, and social services to support safe patient discharges.
• Average daily caseload: approximately 15 cases.
• Work involves inpatient case management with wrap-around services addressing social issues.
• Floating between two nearby facilities (within a 10-mile radius) may be required.
• No weekend shifts required, but optional weekend shifts available.
• Holidays required as scheduled.
• Business casual attire.

Required Qualifications

• Valid Licensed Clinical Social Worker (LCSW) license in Maryland
• Minimum 3 years of Licensed Clinical Social Worker experience, including within the last year
• Minimum 3 years of experience in inpatient case management in an inpatient hospital/acute care setting
• Experience with case management and inpatient hospital/acute care (qualifies first-time travelers)
• Experience working with Epic electronic health record system

Additional Requirements

• Must complete background checks including 7-year criminal search, education verification, and state-specific exclusions.
• Required health documents include physical exam, immunizations (Tdap, MMR, Varicella, Hepatitis B), negative TB test, and drug screening.
• Competency exams and certifications as applicable.
• Must complete facility-specific documents such as confidentiality agreements, dress code review, and job description acknowledgment.
• Two professional references required, including at least one supervisor or charge nurse.
• Employment history verification for the past 5 years.
• Government-issued photo ID required.
• Compliance with facility policies including appearance standards and OSHA questionnaire.

Certifications & Competencies

• Basic Life Support (BLS) preferred.
• Advanced Cardiac Life Support (ACLS) and other certifications as applicable.
• Medication and pharmacology competency exams may be required.
• Facility-specific training and attestations, including Epic training or test-out process.

Notes

• On-call and weekend shifts are not required but may be available.
• Floating between facilities within a 10-mile radius is required as needed.
• Candidate must be prepared to complete all required documentation and training prior to start date.
