Job Opportunity: Hospice Registered Nurse (RN) Case Manager

Location: Aldie, Virginia
Employment Type: Travel/Contract
Contract Length: 13 weeks

Position Overview

• Specialty: Hospice Case Manager (Registered Nurse)
• Patient Load: Approximately 5-7 patients per day (geriatric, end-of-life care)
• Shift: Day shift, 5 days a week, 8 hours per day (8:00 AM – 5:00 PM)
• Guaranteed Hours: 40 hours per week
• Territory: Up to a 60-mile radius from the office; flexibility with territory expected
• Dress Code: Scrubs of any color; jeans are not allowed
• Technology Provided: iPad provided; cell phone use not allowed
• Mileage Reimbursement: Provided at IRS standard rate

Required Qualifications

• Valid RN license in Virginia
• Minimum 1 year of Hospice Palliative Care RN experience within the last year
• BLS certification issued by American Heart Association or American Red Cross
• Associate of Science in Nursing / Associate Degree in Nursing

Compliance and Documentation Requirements

• Government-issued photo ID (Driver’s License or State ID)
• Background checks including:

  • 7-year history check (locations lived/worked/schooled)
  • Virginia State Police background check and sex offender search
  • EPLS/GSA/SAM and HHS/OIG checks within 30 days of start
• Fingerprinting clearance (Florida AHCA Level II fingerprint results required for direct care providers; must be current and cleared prior to start)
• Physical exam within 6 months prior to start, confirming freedom from communicable diseases
• Fit test for N95 mask within 1 year of start (can be completed onsite)
• Immunizations and titers required:

  • TDAP within 10 years
  • Positive titers (or boosters) for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella
• Negative TB test or chest X-ray per agency guidelines
• Negative 10-panel drug screen within 30 days of start
• Annual skills checklist and competency exams as applicable
• Facility-specific forms and agreements including confidentiality, code of conduct, sexual abuse policy, contractor consent, immunization statement, and conflict of interest
• COVID-19 vaccine or facility-specific declination required
• Seasonal flu vaccine or declination required; if declined, mask wearing is mandatory

Additional Notes

• No on-call responsibilities
• Local candidates accepted at travel rates
• Alzheimer’s disease training (1 hour) required for candidates working at PACE facilities
• All documentation must be completed and verified prior to start date
