Licensed Practical Nurse (LPN) - Travel/Contract
Location: Chicago, Illinois
Contract Length: 13 weeks
Job Summary
Seeking a Licensed Practical Nurse (LPN) for a travel/contract position in an acute care correctional facility setting. This role requires flexibility to float within the facility as needed.
Required Qualifications
- Valid LPN/LVN license in Illinois.
- Minimum 1 year of Long Term Care experience within the last year.
- Experience in an acute care setting.
- Experience in an outpatient setting.
- Correctional nursing experience.
- Basic Life Support (BLS) certification issued by the American Heart Association.
- Bystander Intervention Training Certificate.
Work Schedule & Requirements
- Guaranteed 40 hours per week.
- Weekend requirement: Every other weekend (2 weekends per month).
- Return to Office (RTO) policy: Maximum 5 days with no more than 2 consecutive days; no RTO on holidays or adjacent days; no RTO on weekends.
- Mandatory floating within scope of practice to meet facility needs.
Certifications & Competencies
- CPR certification required.
- Medication Test/Pharmacology Competency Exam required within 12 months of start.
- Color Blind Testing required within 12 months of start.
- Competency Exam required within 12 months of start.
Health & Safety Requirements
- Physical exam within 12 months, signed by PA, NP, DO, or MD.
- Fit mask (N95 or equivalent) within 1 year, updated annually. Medical or religious exemptions require documentation.
- Tetanus/Diphtheria/Tdap vaccine within 10 years or signed declination within 12 months.
- Negative TB test (QGold or TSPOT) within 12 months, updated annually. Chest X-ray required if TB positive.
- Required immunizations or titers: Measles, Mumps, Rubella (2 MMR vaccines or positive titer), Hepatitis B series or declination, Varicella (2 vaccines or positive titer).
- Negative 9-panel drug screen within 90 days of start, updated annually. Marijuana testing not required.
- Seasonal flu vaccination or approved medical/religious exemption required during flu season (Oct 1 - Apr 1).
Background & Compliance
- Background check within 12 months covering all locations lived/worked/schooled in past 7 years, including aliases and national sex offender registry.
- EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start.
- Illinois Healthcare Worker Registry verification required for aides and nursing assistants (not applicable for LPN).
- Completion of facility-specific forms and rules of behavior required.
- Non-employee application with PREA clearance must be completed within 24 hours of contract signing.
