Job Summary
Provide medically prescribed physical therapy treatment to patients in a home health setting, focusing on improving/restoring function, increasing strength, relieving pain, and enhancing functional independence. Must demonstrate competencies specific to the patient population served.
Work Schedule
- Shift: 8:00 AM - 5:00 PM
- Hours: 40 hours per week (Full-Time)
- Weekend Requirement: Every other weekend (with a weekday off during rotation week)
- Call Requirement: None
- Float Requirement: None
Coverage Area
- Van Buren, Calhoun, and Kalamazoo Counties, Michigan
Required Qualifications
- Michigan Physical Therapy License (MI license)
- Basic Life Support (BLS) certification (AHA only)
- Home health experience required
- Current Michigan driver's license, daily availability of a car for business use, and proof of insurance
- Must meet all background checks and compliance requirements (see below)

Clinical Duties
- Interpret and implement referrals
- Evaluate patient functional abilities/disabilities (range of motion, strength, coordination) using standardized assessments
- Establish functional baselines and treatment goals
- Participate in interdisciplinary team discussions, rounds, and clinics
- Provide patient, family, and community education
- Conduct individual and/or group physical therapy activities per treatment plan
- Design and fabricate splints and assistive devices as needed
- Participate in program development and department process improvement
- Serve as a resource and provide in-services within the rehabilitation system
- Cross-train and provide coverage in other rehabilitation departments as needed
Administrative Duties
- Document evaluations, treatment plans, treatments performed, and patient progress in the electronic health record (EHR) timely and accurately
- Communicate any limitations preventing timely documentation to leadership
Required Certifications & Compliance
- Basic Life Support (BLS) - AHA only
- Physical exam within 1 year of start
- Criminal background check (lifetime search, including SSN trace, national/statewide searches, education verification, motor vehicle record, etc.) within 45 days of start
- MI LARA fingerprinting within 45 days of start (required for home health positions)
- Drug testing: 10-panel including Marijuana/THC within 30 days of start
- Immunizations and health documents:
- Tetanus/Diphtheria/Tdap (proof of at least one prior Tdap vaccine)
- Negative TB test or chest x-ray within 30 days of start
- Annual TB questionnaire
- Rubeola (Measles), Mumps, Rubella: 2 vaccines or positive titer required
- Hepatitis B: titer, series, or facility declination required
- Varicella: 2 vaccines or positive titer required
- Seasonal Flu vaccination or approved exemption (for candidates starting after Nov 1)
- Orientation Safety Manual Acknowledgement
- Medication Test/Pharmacology Competency Exam (passing score within 1 year prior to assignment)
- Competency Exam in specialty (passing score within 1 year prior to assignment)
- Personal or auto insurance proof (updated every 6 months)
- Code of Conduct review and signed acknowledgment
- Appearance Standards Policy review
Additional Requirements
- Complete application/resume with full 7 years work history, no gaps
- One professional reference from a supervisor or manager
- MI Central Registry Clearance (if applicable for pediatric or specific placements)
