Job Opportunity: PT Home Health (Therapy/Rehabilitation)

Experience & Licensure Requirements

• Years of Experience Required: Minimum 1 year in specialty
• State License: Maryland PT State License required
• Certifications:

  • BLS (Basic Life Support) required

Documentation & Compliance Requirements

Background Checks

• 7-year criminal background check within 1 year of start, updated annually
• EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start and updated for renewals
• Fingerprinting (Maryland CJIS/DSS/FBI) required for behavioral health units or campuses; must be completed in Maryland at least 5 business days before start date with clear results before starting

Certifications (as applicable)

• BLS (AHA only) - must be current and not expiring within 30 days of start
• ACLS, PALS, NRP, TNCC, NIH Stroke Certification, CPI, and others as applicable to specialty or unit
• State License primary source verification required
• NURSYS verification required for RNs

Health & Immunization Requirements

• Physical exam within 1 year of start, updated annually
• Fit mask testing within 12 months of start; no religious or medical exemptions accepted without facility approval
• Tetanus/Diphtheria/Tdap vaccine within last 10 years (must include Pertussis)
• Negative TB test (QuantiFERON-TB Gold or TSPOT within 90 days) or 2-step TST; chest x-ray if history of positive TB
• Immunizations or positive titers required for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Varicella
  • Hepatitis B (positive quantitative titer within 5 years; series must be repeated if low titer)
• Drug screening: 11-panel including cotinine/nicotine and marijuana within 30 days of start
• Seasonal flu vaccination required annually (no declinations without approved exemption)
• Visual acuity exam within 6 months of start, repeated annually
• Color blind exam within 5 years of start

Other Requirements

• Proof of 2 clinical references or hospital evaluations
• Completed employment application/resume
• Skills checklist updated annually (if applicable)
• Radiation badge and attestation of prior radiation exposure if working in radiology-related areas

Facility Specific Notes

• Smoke/nicotine free campus; no exceptions
• Behavioral health fingerprint confirmation and CPS background check required for behavioral health units
• Fit test medical or religious exemptions require facility-approved forms and may result in PAPR training if approved
• Fingerprinting must be scheduled and completed in Maryland with government-issued photo ID and required forms
• Facility requires timely submission of personal information (address, DOB, email, phone) within 24 hours of signing contract
