Shift: Dayshift, 07:30 AM - 04:00 PM, Monday to Friday
Contract Length: 13 weeks

Job Description

• Registered Echocardiographer position
• Provide echocardiography services during day shift hours

Required Certifications and Credentials

• American Heart Association BLS Certification (front and back images required)
• License/Certificate Verification (must be dated within 30 days of start)
• Fit Test
• Trinity 10 Panel Drug Screen (completed within 30 days of start; excludes THC)
• Trinity Code of Conduct Form
• Trinity I9
• Confidentiality of Patient & Proprietary Information and System Security Agreement
• Integrity and Audit Services Form
• Specialty Specific Competency Testing (completed within the last year)
• Color Vision Test
• COVID-19 Vaccine (proof of booster, declination or exemption; declination not accepted in CA or IL)
• MMR Vaccination (proof of 2 vaccinations or IgG titer)
• Hepatitis B Vaccination (series must be started prior to start date; titers not accepted)
• Influenza Vaccine (proof or declination; declination not accepted in CA or IL)
• 2 Step PPD OR IGRA (Quantiferon/Tspot) (with chest x-ray if positive, per regional requirements)
• Varicella Vaccination (proof of 1 vaccination or IgG titer)
• Physical/Health Statement (exam within last year confirming fitness to work)

• Resume and Skills Checklist (within the last year)
• Two Professional References
• Conflict of Interest Disclosure Form
• Criminal Background Check (Federal, Nationwide, County for last 7 years, including SSN verification; dated within 30 days)
• Education Verification (highest level of academic education verified)
• EPLS (Excluded Parties List System) Check (dated within 30 days)
• GSA/OIG Exclusion Check (dated within 30 days)
• Joint Commission CORE Exam (within the last year)
• NSO (National Sex Offender) Check (dated within 30 days)
• System Access Form (completed and uploaded one week before start)
• Skills Checklist (provided by agency for clinical contractors)

Notes

• All documentation must be uploaded to the designated system prior to onboarding.
• Drug screen panel excludes THC.
• Declinations for vaccines are not accepted in California or Illinois.
• Physical and health certifications must be current within one year.
