Job Description

• Day shift Registered Nurse for the Medical ICU unit.

Required Experience

• Specialty: RN MICU

Credentials & Compliance Requirements

Personal Health History

• COVID-19 Vaccine: 2 vaccines plus booster required or completed declination
• MMR: Proof of 2 vaccines or positive titer showing immunity
• Fit Test: Annual respirator fit test required prior to start
• Hepatitis B: Required, no declinations accepted
• Influenza Vaccine: Required during flu season (Nov 1 - Mar 31), no declinations accepted
• TB Test: Within 1 year of start date
• Varicella: Vaccination or titer showing immunity
• Tdap: Required, no declinations accepted
• Positive PPD with Negative Chest X-Ray: Optional
• Physical/Health Statement: Annual, must state clear to work, completed within 60 days of start

Professional Credentials

• Driver's License, State ID, or Passport required
• Prime OIG clearance required
• Prime Criminal Background Check: National check within 1 year of start date
• Prime SAM/EPLS: Must verify exclusion status
• Prime Form I-9 (Employment Eligibility Verification): Agency attestation accepted
• Prime Agency Attestation: Completed after all credentials uploaded
• Prime Badge Photo: Optional, facility specific
• Prime Job Description: Signed by employee
• Prime Resume: Required at time of offer acceptance, must be up to date
• References: 2 required, including 1 from a manager or direct supervisor
• Name Verification Affidavit: Optional, for name discrepancies

Testing & Exams

• Prime Core Competencies Exam: Must score 80% or higher
• Prime Skills Checklist: Required
• Prime Specialty Exam: Must score 80% or higher

Certifications

• DCI/BVP (Workplace Violence Prevention): Required, can be completed via Relias or similar

Drug Screening

• Prime Drug Screening: Minimum 10-panel test including THC, within 60 days of start
• For CA locations: 10-panel urine drug screen excluding THC plus psychoactive THC saliva test

Additional Notes

• Prior travel experience is not explicitly required for this position.
• All onboarding requirements must be completed prior to start date.
• Declinations for vaccines and tests are generally not accepted for this assignment.
