Job Details
- Employment Type: Long-Term Assignment (LTA)
- Contract Length: 13 weeks
- Shift: 7:00 AM – 5:30 PM (10.5 hours)
- Weekly Hours: 40 hours per week
- Schedule: Variable weekly schedule with every other Saturday coverage including on-call
- Work Locations: Primarily two facilities; must be willing to assist at other facilities when census is low at primary site
- Transportation: Reliable transportation required
Required Qualifications
- Valid RN license in Ohio
- Minimum 1 year of dialysis experience, with dialysis experience within the last year
- BLS certification issued by the American Heart Association
- ACLS certification issued by the American Heart Association
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
Health and Safety Requirements
- Immunizations and Titers:
- MMR: Positive titers within last 15 years or approved booster protocol
- Varicella: Positive titers within last 15 years or approved booster protocol
- Hepatitis B: Proof of full series or partial series with signed declination; positive titers accepted within 15 years
- Tdap: Vaccination within last 10 years (no declinations accepted)
- Influenza Vaccine: Proof required during flu season (October–March); declination requires mask-wearing and completion of specific forms
- Tuberculosis Testing: 2-step PPD or IGRA test with negative results within last 6 months; if positive, clear chest x-ray and current TB questionnaire required
- Fit Test: Annual respirator fit test completed within 1 year prior to start (specific approved models)
- Physical Exam: Pre-employment physical exam within 1 year confirming ability to work without restrictions
- Drug Screen: 10-panel drug screen within 60 days prior to start; instant drug screens not accepted
Background and Verification
- Criminal Background Check: Within 6 months prior to start, including Social Security trace, county searches for last 7 years, national sex offender registry, and national criminal sweep
- Fingerprinting: Ohio residents must complete Ohio BCI&I fingerprint check and FCRA-compliant national database check; non-Ohio residents require FBI fingerprint check
- Employment Verification: Verification of employment within last 5 years through third-party; attestations not accepted
- Education Verification: Highest level of education verified through third-party; international education requires equivalency documentation
- I-9 Attestation: Proof of eligibility to work in the U.S.
- OIG and Federal Exclusion Checks: Completed within 6 months prior to start
- Social Security Trace: Recommended if not included in background check
