Job Description

  • Specialty: Home Health (North Napa Valley)
  • Average Patients per Day: 5-7
  • Visits per Day: 6
  • Minimum Productivity: 30 points weekly
  • Types of Cases: Geriatric
  • Guaranteed Hours: 40 per week
  • Territory: Up to 60-mile radius from hub/office
  • On-call: Yes, every 3rd to 4th weekend
  • Dress Code: Scrubs (any color), no jeans
  • Equipment Provided: iPad
  • Mileage Reimbursed at IRS standard rate
  • Cell phone use: Not allowed
  • Flexibility with territory expected
  • Timely documentation and communication required
  • First-time travelers accepted

Required Qualifications

  • Discipline: Physical Therapist (PT)
  • Minimum Years of Experience: 1 year (recent)
  • Certifications: BLS (Basic Life Support) required
  • License: Valid Physical Therapist license in hand

  • Fit mask test (N95 accepted)
  • Negative TB test or chest X-ray within 1 year (or TB questionnaire if applicable)
  • Immunizations or titers for:
    • Rubeola (Measles)
    • Mumps
    • Rubella
    • Hepatitis B
    • Varicella
  • Drug screening (9 panel) within last 12 months
  • Seasonal flu vaccination or facility-specific declination form required
  • COVID vaccination or facility-specific declination required

Documentation and Verification

  • 2 professional references within 2 years of start
  • Full 7 years of work history with no gaps required on application
  • Annual skills checklist and competency exams as applicable
  • Proof of current auto insurance required (due to driving responsibilities)
