Job Opportunity: Home Health LVN / LPN

Location: Portland, Oregon (West Portland and surrounding areas including Beaverton)
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Details

• Start Date: ASAP
• Shift: 8-hour day shifts only (no 4x10 or 3x12 shifts)
• Patient Ratio: 1:1 (may visit up to 6 patients per day; 30 productivity points standard)
• Experience Required: Minimum 1 year of skilled home health experience

Certifications Required

• BCLS (Basic Cardiac Life Support) - Must be current
• COVID-19 primary series vaccination plus one booster required

• Charting System: EPIC Remote Client (laptop) and Rover (phone app)/Oasis

Licensure & Background Requirements

• Must have valid Oregon nursing license (pending license accepted)
• Background check within 45 days of start covering all locations lived/worked in past 7 years
• Various state-specific background and abuse/neglect searches as applicable
• Education verification required
• Drug screen (10 panel) within 90 days of start
• Physical exam within 1 year of start, signed by MD, NP, PA, or DO
• Immunizations and health documents required including:

  • TDAP (within 10 years or declination)
  • Negative TB test or chest x-ray within 12 months
  • MMR (Measles, Mumps, Rubella) vaccination or positive titer
  • Hepatitis B vaccination or signed declination
  • Varicella vaccination or positive titer
  • Seasonal flu vaccine or declination during flu season
  • COVID vaccine or declination

Additional Compliance & Training

• Must complete Providence Health and Services required WorkRamp training modules prior to start (including annual regulatory requirements)
• EPIC training modules required and must be completed within a week of orientation
• Respirator fit testing required for travel candidates (updated annually)
• Medication and competency exams as applicable
• References: Minimum 2 supervisor references covering 1 year of employment in a comparable specialty/setting within the last 3 years
• Employment application covering at least last 5 years required

Special Notes

• Facility requires primary series COVID vaccination plus one booster
• Traveler will have a maximum daily driving radius of approximately 60 miles (60-90 minutes total drive time)
• No weekend or on-call shifts required
• Locals within 50-75 miles considered local and paid the same rate
• Facility may call off up to one shift every two weeks
