Overview

• Travel/Contract assignment in Hawthorne, New York
• Home health setting
• 13-week contract
• Day shift: Monday–Friday, 8:00 AM–5:00 PM
• 40 hours per week

Required Qualifications

• Valid RN license in New York
• Associate of Science in Nursing / Associate Degree in Nursing
• BLS issued by American Heart Association or American Red Cross
• One year of home health experience within the last year
• One completed travel assignment
• Experience working with adult patients
• Hospice experience
• Driver's License
• Experience with Homecare Homebase
• Experience with OASIS

Preferred Qualifications

• Prior HCHB experience outside the 6-month window may be considered if retraining is completed during compliance

Assignment Details

• Territory: Westchester
• Average daily caseload: 5 to 8 patients
• Weekly productivity expectation: 30 points
• Typical daily volume: 5 to 10 patients
• Mileage reimbursement at the IRS standard rate
• Travel between patient homes is reimbursable
• Travel to the first patient and from the last patient is not billable
• No guaranteed hours
• Weekend rotation: approximately once every 4 to 6 weeks
• On-call: during the weekend worked
• Locals are accepted at the same rate
• Scrubs required

Additional Notes

• Case types may include Start of Care, Wound Care, Follow-up visits, and other adult home health or hospice services
• If patient census is low, there may be floating to a nearby clinic for additional hours
• Weekend requirements and on-call responsibilities vary by location and profession
