Required Qualifications

• Surgical Technologist with at least 1 year of experience, including experience within the last year
• Certified Surgical Technologist (CST) certification
• Advanced Cardiac Life Support (ACLS) certification

Required Certifications

• BLS (Basic Life Support): AHA Instructor/Provider/RQI required.
• Other Certifications:

  • National certification verification prior to start (as applicable).
  • Orientation Safety Manual Acknowledgement required.

Required Competencies

• Medication Test/Pharmacology Competency Exam
• Competency Exam relevant to profession/specialty (renewed annually)
• Dysrhythmia/EKG Competency Exam (not required for Psych, OR, Rehab, or Allied positions)
• Core Competency Parts I, II, and III

Background and Health Requirements

• Comprehensive background checks including criminal, sex offender, fugitive, and employment verifications.
• Drug testing within 30 days of start using a specific 9-panel bundle (no THC) with expanded opiates and oxycodone.
• Annual physical exam or physician statement confirming fitness for work.
• Immunizations and health screenings including:

  • Tetanus/Diphtheria/Tdap (valid within 10 years)
  • Negative TB test (QuantiFERON blood test within 1 year) or chest X-ray if positive TB history
  • MMR (Measles, Mumps, Rubella) proof of vaccination or positive titer
  • Hepatitis B series, titer, or declination
  • Varicella vaccination or positive titer
  • Seasonal flu vaccination or approved medical/religious exemption during flu season
• Fit mask testing completed onsite; PAPRs not accommodated.

Documentation and Compliance

• Unexpired government-issued photo ID (e.g., driver's license or state ID).
• I9 attestation completed by agency representative.
• Employment application/resume with education and work history.
• Two professional references within 1 year of start date (one must be a supervisor or higher).
• Work history verification covering 7 years.
• State-specific registry checks as applicable (e.g., West Virginia APS/CPS, Kentucky Vulnerable Adult Maltreatment Registry).
• Facility-specific job description signed and dated annually.
• Completion of employee health attestation and required health documentation uploads prior to start.
