Physical Therapist - Outpatient
Location: Bend, Oregon
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Description
- Outpatient oncology rehabilitation setting
- Caseload primarily includes cancer patients, neurologic, and geriatric patients
- Lymphedema experience preferred but not required
- Will accept 4x10 or 5x8 work schedules
- Average daily census: 9 patients
- Epic experience preferred
- First-time travelers are welcome
- Clinician may float within scope to meet facility needs, including other facility locations if needed
- Locals not accepted unless residing outside a 75-mile radius (proof of address required)
- Pending license accepted (can be obtained within 48 hours)
- No on-call required
- Wear your own scrubs; no required color
Required Qualifications
- Valid Physical Therapist license in Oregon (pending license accepted)
- Minimum 1 year of experience as a Physical Therapist, including experience within the last year
- Minimum 1 year of experience in outpatient physical therapy, specifically outpatient oncology rehabilitation
- Basic Life Support (BLS) certification required
Background & Compliance Requirements
- 7-year criminal background check required within 90 days of start, updated annually
- EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start
- Education verification and diploma/transcript required
- Primary source verification of state license required
- Photo ID for badge required within 48 hours of signing
- Various facility-specific confidentiality, compliance, and emergency contact forms required within 48 hours of signing
Health & Safety Requirements
- Physical exam within 1 year of start
- TDAP vaccine within 10 years or declination accepted
- Negative TB test or chest X-ray as applicable
- Annual TB questionnaire required
- Proof of immunity or vaccination for Measles, Mumps, Rubella, Hepatitis B, and Varicella (titers or vaccines)
- Negative 12-panel drug screen within 90 days of start
- Seasonal flu vaccination or declination required during flu season
- COVID vaccination preferred but not required; declination or exemption required if unvaccinated
- Respiratory safety clearance required annually
Additional Information
- Maximum of 7 total days off allowed during the 13-week assignment
- Skills checklist required annually
