Occupational Therapist Travel/Contract Job - Lancaster, California

Job Details

• Discipline: Therapy/Rehabilitation
• Position: Occupational Therapist (OT)

• Employment Type: Travel/Contract
• Assignment Length: 13 to 26 weeks preferred
• Start Date: ASAP
• Shift: 5 days per week, 8-hour day shifts
• Schedule Notes: May include occasional weekend coverage and floating
• Weekly Hours: 36 hours per week
• Cancellation Notice: 14 days

Requirements/Qualifications

• Valid Occupational Therapist license in California
• Minimum 1 year of Occupational Therapist experience within the last year

Required Certifications & Licenses

• BLS (Basic Life Support) certification from American Heart Association or American Red Cross
• ACLS (Advanced Cardiac Life Support) certification preferred
• PALS (Pediatric Advanced Life Support) preferred

Required Documentation & Compliance

• Government-issued photo ID (Driver’s License or State ID)
• Emergency Contact Form prior to start
• Background checks including 7-year work/school history, sex offender registry, and federal exclusions (updated annually)
• Physical exam within 1 year of start date
• Negative TB test or chest X-ray within 1 year (with specific guidelines if prior positive)
• Immunization or titer proof for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B (titer, declination, or vaccine series)
  • Varicella (titer, vaccine, or history of chickenpox)
• Drug screening: 9-panel drug test within 1 year of start
• Seasonal flu vaccination or declination during flu season (Oct 1 - Mar 31)
• Professional references:

  • 2 for travel, allied, or per diem acute care roles
  • 1 for per diem non-acute care roles
• Annual competency exams and skills checklist as applicable
• Medication test/pharmacology competency exam as applicable
