PT Home Health - Travel/Contract Opportunity

Location: San Ramon, California
Contract Length: 13 weeks

Job Details

• Specialty: Therapy/Rehabilitation (Physical Therapy)
• Setting: Home Health
• Shift: Day shift, 8 hours per day (8:00 AM - 4:30 PM), 5 days per week
• Weekly Hours: 40 hours (Guaranteed 36 hours weekly, with expectation to see 5-6 patients per day)
• Patient Ratio: 1:1, may visit up to 6 patients per day
• Weekend Work: Optional weekend patient visits to meet productivity goals
• On-Call: Not required
• Locals Accepted: Yes
• Start Date: ASAP (Compliance process takes about 2 weeks)
• Travel Radius: Occasional travel up to 50+ miles; system groups clients to minimize drive time
• Transportation: Own transportation required

Required Qualifications

• Valid Physical Therapist license in California
• Minimum 1 year of Physical Therapist experience within the last year
• Experience in a Home Health setting
• Current Basic Life Support (BLS) certification

Documentation & Compliance Requirements

• Government-issued photo ID (Driver’s License or State ID)
• Emergency Contact Form
• I9 Attestation
• Background Checks including 7-year work/living history, sex offender registry, EPLS/GSA/SAM, and HHS/OIG checks (updated annually)
• Certifications: BLS (required), ACLS and PALS recommended but not mandatory
• State License Verification (primary source online verification)
• Competency Exams as applicable (medication/pharmacology competency)
• Personal or Auto Insurance (must be current throughout assignment)
• Auto Registration (must be current throughout assignment)
• Physical Exam within 1 year, cleared to work without restrictions
• Immunizations and Titers:

  • Tdap within 10 years or annual declination
  • Negative TB test or chest X-ray within 1 year
  • Measles, Mumps, Rubella (2 vaccines or positive titers within required timeframes)
  • Hepatitis B (titer within 5 years, vaccine series, or declination)
  • Varicella (2 vaccines or positive titer)
• Drug Testing: 10-panel drug screen within 1 year
• Flu Vaccine or Declination (required year-round)
• COVID-19 Vaccine documentation or facility-specific declination form required
• References:

  • 2 professional references for travel, allied, or per diem acute care
  • 1 professional reference for per diem non-acute care
• Work History Verification: 1 year employment verification related to profession
• Annual Skills Checklist

• Documentation tools will be provided and must be returned
• Timely communication and documentation are required

This position is ideal for experienced Home Health Physical Therapists with prior travel contract experience who are licensed in California and able to manage a full patient load with reliable transportation.
