Physical Therapist - Home Health (Travel/Contract)

Location: Marysville, California
Contract Length: 13 weeks
Shift: Day, 5 days per week, 8 hours per day (7:00 AM - 3:00 PM)

Job Summary

• Plan and conduct treatment programs to restore and improve physical, social, and mental functions.
• Serve as co-leader in interdisciplinary patient care groups.
• Complete patient evaluations and develop individualized treatment and discharge plans.
• Provide therapy interventions using appropriate clinical techniques and protocols.
• Supervise assigned therapy assistants.
• Complete comprehensive and timely clinical documentation.

Required Qualifications

• Valid Physical Therapist license in California.
• Doctor of Physical Therapy (DPT) degree required for individuals licensed on or after January 2016.
• Current Basic Life Support (BLS) certification.
• Experience in supervisor roles.

Experience

Compliance and Documentation Requirements

• Background check covering the last 7 years, including Social Security trace, county/state/federal court jurisdictions, OIG, OFAC, SAM, FACIS 3, National Sex Offender, and DMV/MVR if driving is required.
• Drug testing within 30 days before start, including multiple substance panels.
• Physical exam within 90 days of start (not required annually).
• Immunizations and/or proof of immunity or declinations required for:

  • COVID-19 vaccine (with medical or religious exemptions accepted).
  • Flu vaccine or facility-specific declination/exemption during flu season (10/31 - 04/30).
  • Tetanus/Diphtheria/Tdap within 10 years or positive titer (declinations accepted).
  • Tuberculosis screening (QGold/TSpot or 2 PPDs within 30 days of start, updated annually). Chest X-ray if history of positive TB.
  • Measles (Rubeola), Mumps, Rubella, Varicella, Hepatitis B (positive titer, vaccination series, or signed declination).
• N95 mask fit test preferred prior to start; onsite fit testing possible with authorization.

Additional Requirements

• Two clinical references verified within one year of start.
• Complete employment application/resume including 7-year work history with locations and gaps.
• Unit-specific skills checklist required.
• Passport-style photo for badge with simple background and high resolution.
• Signed confidentiality agreement, waiver of benefits acknowledgment, and intellectual property rights assignment prior to start.
• Completion of contractor compliance education form prior to hard document due date.
