Job Details
- Shift: Day shift, 5 days a week, 8 hours per day (8:00 AM - 4:30 PM)
- Typical Caseload: 1:1 patient ratio, may visit up to 6 patients per day
- Productivity Standard: 30 productivity points per day
- Weekend Work: Not required
- On-Call: Not required
- Floating: Not required
- Block Schedule: Not accommodated
- Locals: Accepted within 50-75 mile radius
- Pending License: Accepted
- Guaranteed Hours: Facility may call off up to one shift every two weeks
- Transportation: Traveler must have own transportation; daily travel radius up to ~60 miles (60-90 minutes total drive time)
- COVID-19 Vaccination: Primary series plus one booster required

- Attire: Business casual; scrubs allowed but not required; no jeans
- Charting System: EPIC Remote Client (laptop) and Rover app (phone)

Required Qualifications
- Valid Physical Therapy Assistant license in Oregon (pending licenses accepted)
- Minimum 1 year of Physical Therapy Assistant experience within the last year
- Experience in a Home Health setting
- Basic Life Support (BLS) certification
Health & Safety Requirements
- Physical Exam: Within 1 year of start, signed by MD, NP, PA, or DO, indicating ability to work with or without accommodation
- Immunizations & Testing:
- COVID-19 vaccination (primary series + booster)
- Flu vaccine during flu season or signed declination
- Tetanus/Diphtheria/Tdap within 10 years or declination
- Measles, Mumps, Rubella (2 MMR vaccines or positive titer or declination)
- Varicella (2 vaccines or positive titer or declination)
- Hepatitis B vaccination or signed declination
- Negative TB test or chest X-ray within 12 months, updated annually
- Drug Screen: 9-panel drug screen within 12 months of start
- Respiratory Safety Clearance: Required annually for respirator use (travel only)
- Fit Mask Testing: Required annually for travel candidates, completed onsite if no valid record within 1 year
Background & Compliance
- Background Check: Within 90 days of start, covering all locations lived/worked in past 7 years; includes SSN trace, death master index, nationwide checks, motor vehicle record, and Oregon Abuse/Neglect search if applicable
- E-Verify: Required
- Education Verification: Required for all candidates
- State-Specific Searches: Required for Oregon and Washington border facilities as applicable
- WA Multi-State Licensed Nurses Demographic Attestation: Required if working with a compact license in Washington
Additional Requirements
- Transportation: Must have reliable personal transportation due to travel between patient homes
- Scheduling: No accommodation for 4x10 or 3x12 shifts
- Float Policy: Not required to float between units for this position

- First-time travelers are accepted
- Locals within 50-75 miles are considered and accepted at the same rate
- Facility may call off up to one shift every two weeks
