Occupational Therapist - Home Health (Travel/Contract)

Location: Riverside, California
Contract Length: 13 weeks

Job Details

• Employment Type: Travel/Contract
• Specialty: Therapy/Rehabilitation (Occupational Therapy)
• Schedule: Monday to Friday, 8:00 AM to 5:00 PM (Day shift, 5x8 hours)
• One weekend day on call required
• Work setting: Home Health
• Locals are eligible to apply
• Mileage reimbursement between patients at IRS standard rate
• Hours guaranteed if 25 or more visits are completed weekly (25 visits = 40 hours)
• COVID exemptions permitted

Required Qualifications

• Valid Occupational Therapist license in California
• Experience with OASIS

Required Documentation & Compliance

• Government-issued photo ID (Driver’s License or State ID)
• Valid passport copy before start
• I9 attestation for employment clearance
• Standard background check (7-year search including SS trace, county, nationwide, sex offender) updated annually
• EPLS/GSA/SAM and HHS/OIG checks required annually
• Physical exam within 12 months of start
• Immunizations and health requirements:

  • TDAP vaccine within past 10 years (no declinations accepted)
  • Negative TB test or chest X-ray within 1 year (or annual TBQ and CXR if past positive)
  • Measles, Mumps, Rubella proof of immunity or vaccination for candidates born in 1957 or later (no declinations accepted)
  • Hepatitis B titer or vaccine series (declination accepted if within 1 year)
  • Varicella immunity or vaccination (no declinations accepted)
  • Negative 9-panel drug screen within 1 year (instant drug screen acceptable)
  • Seasonal flu vaccination/documentation required year-round (declinations accepted)
  • COVID vaccination proof or medical/religious exemption required prior to start
• Photo for badge (passport style, shoulders up, front facing, plain background)
• Personal or auto insurance and auto registration required and must be kept current during assignment
• Completion and signature of facility-specific training content and hand hygiene policy acknowledgments prior to start

Additional Requirements

• Two professional references required
• Employment application and annual skills checklist required for clearance
• Medication test/pharmacology competency exam and other competency exams as applicable (minimum passing score 80% or meet/exceed Angoff score)

• Candidates with pending California license must have applied before submission

• Home Health experience is preferred but candidates without it will be considered
