• Monday–Friday, 8:30 AM–5:00 PM

Assignment Length

• 15 weeks

Setting

• Critical access hospital
• 90% outpatient, 10% inpatient
• General orthopedics, neurology, and gait

Required Qualifications

• Doctoral degree in Physical Therapy, unless licensed prior to 2016 under the grandfathered licensure standard
• Active Illinois Physical Therapy license, or proof that the application process has been initiated
• BLS certification
• CPR certification within 90 days of hire
• Must be able to provide a negative drug screen for all panels

• Middle initial
• Requested time off

Additional Notes

• Illinois license waiver accepted
• Local rate applies within 50 miles of the facility
• 12 hours of non-billable orientation
• Call-off allowance is equivalent to 12 hours every 13 weeks without penalty
