Registered Nurse – Home Health/Hospice

Location: Bristol, Connecticut
Contract Length: 13 weeks

Job Details

• Specialty: Home Health/Hospice
• Shift: Monday to Friday, 8:00 AM – 4:30 PM (40 hours/week guaranteed)
• Call Requirements: Every 4th weekend on call (flex a day from the M-F shift)
• Weekend/Holiday: Weekend call requires callback within 20 minutes and patient visit within 30 minutes; holiday rotation with permanent staff
• Patient Population: Adults and geriatrics; no pediatrics
• Common Diagnoses: Medication management, CFH, COPD, post-surgical care, orthopedic total joints, CVA, dementia, hospice care
• Typical Caseload: Average 5-6 visits per day, using a weekly point system (start of care = 3 points, general visit = 1 point)
• Travel Requirements: Travel within approximately a 20-minute radius in an urban area
• Orientation: 1 week with preceptor, Monday-Friday 8:00 AM – 4:30 PM, includes some overnight on-call responsibilities
• Dress Code: Clean, professional attire of any color

Required Qualifications

• Licensure: Must be a licensed RN in Connecticut, fingerprinted as required by the state
• Certifications: BLS certification required
• Experience: Minimum 1 year of home care or hospice nursing experience (higher experience requirement takes precedence)
• Preferred Skills: Experience with Meditech home care EMR and Oasis documentation highly preferred
• Additional Requirements: Valid driver’s license, reliable car, and auto insurance

Job Responsibilities

• Conduct patient evaluations, treatments, and develop plans of care
• Manage common home health/hospice patient needs without ventilator support (oxygen only)
• Collaborate with home health/hospice aides and administrative staff available 24/7
• Self-scheduling flexibility within the Monday-Friday shift and weekend rotation

• Call involves timely response and patient visits on weekends
